1. Be sure to fill in your complete name and correct current address. Your
courses will be sent to the address you enter. 2. Have you already taken
your Care Courses and would like to be reimbursed by SUNY? Please fill out
your SUNY paperwork and send into SUNY. When they return your completed
paperwork to you, please send it to us with your receipt from
your Care Courses order.
3. Your start date should
be about a month after you submit your paperwork. Choose a date about
30 days from the day you mail it in to SUNY as it takes about a month to get
your paperwork processed.
4. Your completion date should be realistic. How many hours are you
ordering? How long will it take you to finish this many training
hours? Be sure the finish date is in this calendar year. Many
students choose a date late in the year just to be safe (November or early
December).
5. SUNY will require that you pay them back for any courses you don't
complete within this year, so please finish your courses! Please read your
contract with SUNY to ensure you understand what you're liability is.
6. Please note: Because E-Courses are
delivered by an online delivery system that is activated by online payment with a
credit card or Paypal, EIPs cannot be used for E-Courses.
